In return, we will provide you with your user name and password to securely log on to the Premier Pain Patient Portal. After you log on, please enter your medical history. You will find instructions on the Portal’s Welcome Page.

You must still download the Portal New Patient Form by clicking here, complete the remaining documents, and bring them with you to your next appointment.  This will provide you with the best service possible during your visit.

We are proud to inform you that our practice now offers the ability to review the most important aspects of your pain management care online, through Premier Pain’s new Patient Portal.  The Patient Portal enables our patients to communicate with our practitioners and staff members easily, safely, and securely via the Internet. Through the Patient Portal, you are able to:

  • Ask questions of doctors, nurses, and staff members
  • Request prescription refills and referrals
  • View your personal health record

If you are unable to get access to the patient portal, we request that you then print and fill out the Non-Portal New Patient Form .
